    Reduced traffic exposure due to COVID-19 lockdowns may have resulted in changes to both the frequency and severity of motor vehicle crashes. In order to better understand the impact of COVID-19 lockdowns on traffic safety outcomes in US DOT Region 6, we explored the role of vehicle, user, and built environment factors on fatalities, injuries, and total crashes before and during COVID-19 lockdowns in Oklahoma and Texas. Findings suggest that while crash counts in Region 6 were 45%-50% lower during the COVID-19 lockdowns than the previous five-year averages, crashes were more severe. Fatal pedestrian crash counts decreased 3.9% during COVID-19 and fatal bicyclist crash counts increased 22.0%. These Region 6 outcomes were worse than the US as a whole. COVID-19 crashes were more likely single-vehicle fixed-object or rollover crashes involving unsafe speeds. In Texas, suburban areas saw the most crashes before and during COVID-19. Rural Texas crashes were most likely to result in a serious or fatal injury, and that likelihood got worse during the COVID-19 lockdowns.

    This interpretive study explored participants' perceptions of their child's involvement in Equine-Assisted Activities and Therapies (EAAT). EAAT is implemented with a horse and is based on the notion that interacting with the horse has positive benefits. Such activity is nearly always reported to have positive effects on the child. Few studies have investigated the perceptions of the parents of their children's participation in horseback riding activities. The purpose of this study is to determine how parents and/or guardians perceive how EAAT impacts their young riders, and the epiphanic ways in which the parents view those changes. Seven parents of participating children were purposefully sampled. Riders included five girls (5-10 years old) and six boys (6-16 years old) with cognitive and/or emotional disabilities including Post-Traumatic Stress Disorder (PTSD), Autism Spectrum Disorder, Down syndrome, and more. Findings were analyzed through an interpretive lens of epiphany. Participants spoke of the positive ways in which they and their children were accepted, and discussed improvements in children's strength, social development, and specific individual needs. Parents' views of the impact on their children are essential to improving service and advocacy for their children.

    Distracted driving existed since the invention of the automobile; however, the emergence of the cell phone and electronic devices created another source of distraction that affected drivers visually, physically, cognitively, and audibly. Many studies investigated factors that influenced saturation flow and startup lost time at a signalized intersection, but the impact of new distractions, such as electronic devices and in-vehicle entertainment systems, remained less investigated. This study aimed to characterize technology's influence on driver behavior at intersections and the impact of distractions on startup lost time through a field test conducted at three intersections in Texas. This study used observational analysis and hypothesis testing to understand distraction behaviors and their impacts on individual and aggregated startup lost time. On average, 15% of drivers experienced distraction during a red indication, and a cell phone distracted more than 60% of these drivers. However, in vehicles located behind a heavy vehicle at an intersection, 20% of drivers were distracted, and 80% used a cell phone. Statistical analysis showed that distraction caused significantly higher headways and total lost times than non-distraction conditions. More importantly, technology-induced distraction, which created significant intersection delay, was more uncertain and varied from event to event more than non-technology-induced delay. This study showed that technology-induced distractions from the prevalent use of cell phones created an impact on startup lost time and saturation flow, which remained critical to properly determine the phase and cycle lengths.

    Programme interventionists often determine best practices from systematic reviews of the literature. Interpretations of findings from systematic reviews, however, are susceptible to subjective decisions of the authors. Replication of systematic reviews by different authors on the same topic can increase the authenticity of findings, analysis, and interpretations. The purposes of the current paper were to (a) replicate a systematic literature review using the theory of planned behaviour (TPB) to understand the intention of adults living with physical disabilities to participate in PA, and (b) compare the findings with a published systematic literature review on the same topic. CINAHL, ERIC, SPORTDiscus, Medline, Psycinfo, and Pubmed databases were searched for eligible studies. A total of 11 articles were included for the current review. Intention was the strongest predictor for PA behaviour, with attitude and perceived behavioural control were also significant predictors for intention. Subjective norm had less influence but a distinct role shaping intention. A total of six of 11 studies overlapped with the comparison review. Selection of different studies were due to differences in search databases, search strategy, and inclusion and exclusion criteria. Inconsistency was also found in relationships among other components in the theory, with different interpretations made between the two reviews. The current replication review provides practical and research related implications that add to the understanding of the TBP for persons living with physical disabilities and the findings of the comparative review.

    Introduction. Reperfusion therapy by primary percutaneous coronary intervention (PPCI) is generally indicated in patients suffering from acute myocardial infarction (MI) with ST-segment elevation (STEMI). Prior to hospital admission, full ST-segment resolution (fSTR) may occur. Optimal management of such patients with transient STEMI (TSTEMI) is potentially challenging. Our aim was to evaluate the hypothesis that in TSTEMI patients, patency of infarct related artery (IRA) is achieved before PPCI, and to compare the outcome of TSTEMI and STEMI patients during a prolonged follow-up. Material and Methods. Three hundred consecutive adult STEMI patients were referred to catheterization laboratory. In all patients, standard 12 lead ECGs were obtained both at the time of the first medical contact, and on catheterization laboratory admission. Results. TSTEMI occurred in 20 patients (6.7%). Despite fSTR (isoelectric ST segment), occluded IRA was found in 5 of these patients (25%). Pre-PPCI TIMI flow grade 2 was found in 6 TSTEMI patients (30%). Troponin T value at 24 h after symptom onset was lower in the TSTEMI group (1.8±2.5 mg/L vs. 3.6±3.5 mg/L, P=0.008). These patients also had a lower value of brain natriuretic peptide (156.3±119.5 ng/L vs. 438.5±429.0 ng/L, P<0.001) and higher left ventricular ejection fraction (59.9±6.3% vs. 51.6±10.2%, P<0.001). All patients were followed for a median of 5.6 years during which the overall survival did not differ between the TSTEMI and STEMI groups. Conclusion. Primary PCI is strongly recommended in TSTEMI patients because of a relatively high incidence of occluded infarct related arteries. The rate of patients with TSTEMI is relatively low.

    Muscular strength is a critical component of physical health. The aim of this cross-sectional study was to examine age- and sex-based differences in functional strength and rates of referral for education in a large international sample of Special Olympics athletes. A total of 30,358 (male = 19,661) adult (20–69 years old) athlete functional strength records were utilized from the Special Olympics Healthy Athletes database. Descriptive statistics were computed for the following tests: timed sit to stand, partial sit-up, grip strength, and seated push-up. Analyses of variance with Fisher-Hayter pairwise comparisons generally supported age-related decreases in strength among both males and females in this study. Overall, male participants were stronger than the female participants. However, significantly fewer women were referred for education based on their grip strength scores. The rates of referral for education are a concern regarding core- and leg strength for more than one-third of athletes over the age of 30. These findings suggest that a substantial proportion of Special Olympics athletes would benefit from additional strength and conditioning training. peerReviewed

    Football (soccer) is one of the most popular sports for individuals with visual impairments (VI) worldwide but nascent within the United States (US). While there is burgeoning interest toward developing blind football at grassroot and national levels, particularly with the forthcoming 2028 Paralympics, no football-based talent identification or skill assessment has been developed or vetted in the US. Therefore, the purpose of this study was to investigate the immediate measurement properties of scores from a novel football assessment in amateur-skilled adolescents with VI in the US. Adolescents (n = 57) with VI physically completed the Blind Football Skills Test for Adolescents with VI (BFST-AVI) which was comprised of five football-related tasks. Scores were analyzed by item analyses, internal consistency, convergent validity, and construct validity procedures. The results suggested that total and individual assessment scores had acceptable levels of reliability and validity. The BFST-AVI can be adopted immediately by grassroot organizations, national governing bodies, coaches, adapted and general physical educators, and associated practitioners for talent identification or general motor skill assessment purposes for amateur-skilled adolescents with VI.

    Abstract We previously created and characterized a mouse model of congenital erythrocytosis with low erythropoietin (EPO) levels from a gain-of-function mutation of the human erythropoietin receptor gene (mtHEPOR) (Divoky et al. PNAS. 2001; 98:986; Divoky et al. JMM Berl. 2016; 94:597). These mice develop fetal erythrocytosis, followed by transient amelioration of erythrocytosis in perinatal life, and reappearance at 3-6 weeks of age. Similarly, erythrocytosis is observed in heterozygous mtHEPOR patients postnatally but not at birth. We previously reported dynamic changes of the erythron with iron homeostasis during ontogenesis in these mice (Kralova et al. Blood 2017; 130: 170). We observed that while perinatal mtHEPOR mice exhibit relative iron deficiency, aged mice had iron overload. Here, we evaluated developmentally-determined factors associated with hyperactivation of EPOR signaling which could cause a transition from iron deficiency (neonates) to hyperferremia and increased iron deposition (aged mice). To assess the consequences of different levels of EPOR-JAK2-STAT5 signaling, we studied hetero- and homozygous mtHEPOR mice that differ in their severity of erythrocytosis. We found that prenatally and perinatally, mtHEPOR hetero- and homozygous mice have increased erythroferrone (Erfe) transcripts and reduced hepcidin, consistent with the known inverse correlation between Erfe and hepcidin and in accordance with increased numbers of immature erythroid progenitors in the fetal hepatic circulation. At birth, previously normal Epo expression decreased and remained low in adulthood. Iron deficiency, observed in mtHEPOR hetero- and homozygotes at postnatal day 7, was likely related to increased iron consumption by augmented erythropoiesis at this stage. Postnatally, hepcidin levels increased in mutant mice, accompanied by low Erfe induction and iron accumulation in the liver and spleen as reflected by the upregulation of hepatic Bmp6 expression in mature adult (aged ~6.5 months) and old (~16 months) mtHEPOR homozygotes. We hypothesized that this could be a consequence of diminished iron consumption due to a progressive decline of erythropoiesis in mtHEPOR mice, possibly mediated by premature aging of erythroid progenitors with cell-autonomously increased proliferative history and/or increased inflammation. Indeed, young mutant erythrocytes had decreased erythrocyte survival and expression of a senescent marker CD47, an inhibitor of erythrocytes' phagocytosis. Additionally, a progressive decline in the percentage of Ter119-positive bone marrow cells and immature erythroblasts was observed in mtHEPOR hetero- and homozygotes with aging. Clonogenic assays of old mice revealed suppression of early (BFU-E) and late (CFU-E) erythroid progenitors and myeloid bias of hematopoiesis, paralleled by the up-regulation of PU.1 expression, elevation of platelet counts, and an increase in megakaryocytes chiefly in the bone marrow of mtHEPOR homozygotes. Serum levels of inflammatory cytokines did not indicate systemic inflammation; however, induced transcripts of IL-6, Inf-γ, Tgf-β, and Tnf-α, mainly in mtHEPOR homozygotes showed local bone marrow inflammatory stress. These data indicate progressive attenuation of erythroid drive in mtHEPOR homozygotes, and less so in mtHEPOR heterozygotes, paralleled by a decline in hematocrit levels with aging. In response to attenuated erythropoietic activity, iron consumption was reduced in mtHEPOR mice, leading to iron accumulation in the liver and spleen accompanied by markedly increased hepcidin synthesis. Our data suggest that even in the absence of systemic inflammation, albeit with possible paracrine inflammatory signals, known to affect bone marrow remodeling and hematopoietic aging, life-lasting prolonged activation of EPOR-JAK2-STAT5 signaling promoted exhaustion of erythroid progenitors and resulted in an age-related decline of accelerated erythropoiesis in this mouse model of congenital erythrocytosis with human gain-of-function EPOR.     Reduced traffic exposure due to COVID-19 lockdowns may have resulted in changes to both the frequency and severity of motor vehicle crashes. In order to better understand the impact of COVID-19 lockdowns on traffic safety outcomes in US DOT Region 6, we explored the role of vehicle, user, and built environment factors on fatalities, injuries, and total crashes before and during COVID-19 lockdowns in Oklahoma and Texas. Findings suggest that while crash counts in Region 6 were 45%-50% lower during the COVID-19 lockdowns than the previous five-year averages, crashes were more severe. Fatal pedestrian crash counts decreased 3.9% during COVID-19 and fatal bicyclist crash counts increased 22.0%. These Region 6 outcomes were worse than the US as a whole. COVID-19 crashes were more likely single-vehicle fixed-object or rollover crashes involving unsafe speeds. In Texas, suburban areas saw the most crashes before and during COVID-19. Rural Texas crashes were most likely to result in a serious or fatal injury, and that likelihood got worse during the COVID-19 lockdowns.

    This interpretive study explored participants' perceptions of their child's involvement in Equine-Assisted Activities and Therapies (EAAT). EAAT is implemented with a horse and is based on the notion that interacting with the horse has positive benefits. Such activity is nearly always reported to have positive effects on the child. Few studies have investigated the perceptions of the parents of their children's participation in horseback riding activities. The purpose of this study is to determine how parents and/or guardians perceive how EAAT impacts their young riders, and the epiphanic ways in which the parents view those changes. Seven parents of participating children were purposefully sampled. Riders included five girls (5-10 years old) and six boys (6-16 years old) with cognitive and/or emotional disabilities including Post-Traumatic Stress Disorder (PTSD), Autism Spectrum Disorder, Down syndrome, and more. Findings were analyzed through an interpretive lens of epiphany. Participants spoke of the positive ways in which they and their children were accepted, and discussed improvements in children's strength, social development, and specific individual needs. Parents' views of the impact on their children are essential to improving service and advocacy for their children.

    Distracted driving existed since the invention of the automobile; however, the emergence of the cell phone and electronic devices created another source of distraction that affected drivers visually, physically, cognitively, and audibly. Many studies investigated factors that influenced saturation flow and startup lost time at a signalized intersection, but the impact of new distractions, such as electronic devices and in-vehicle entertainment systems, remained less investigated. This study aimed to characterize technology's influence on driver behavior at intersections and the impact of distractions on startup lost time through a field test conducted at three intersections in Texas. This study used observational analysis and hypothesis testing to understand distraction behaviors and their impacts on individual and aggregated startup lost time. On average, 15% of drivers experienced distraction during a red indication, and a cell phone distracted more than 60% of these drivers. However, in vehicles located behind a heavy vehicle at an intersection, 20% of drivers were distracted, and 80% used a cell phone. Statistical analysis showed that distraction caused significantly higher headways and total lost times than non-distraction conditions. More importantly, technology-induced distraction, which created significant intersection delay, was more uncertain and varied from event to event more than non-technology-induced delay. This study showed that technology-induced distractions from the prevalent use of cell phones created an impact on startup lost time and saturation flow, which remained critical to properly determine the phase and cycle lengths.

    Programme interventionists often determine best practices from systematic reviews of the literature. Interpretations of findings from systematic reviews, however, are susceptible to subjective decisions of the authors. Replication of systematic reviews by different authors on the same topic can increase the authenticity of findings, analysis, and interpretations. The purposes of the current paper were to (a) replicate a systematic literature review using the theory of planned behaviour (TPB) to understand the intention of adults living with physical disabilities to participate in PA, and (b) compare the findings with a published systematic literature review on the same topic. CINAHL, ERIC, SPORTDiscus, Medline, Psycinfo, and Pubmed databases were searched for eligible studies. A total of 11 articles were included for the current review. Intention was the strongest predictor for PA behaviour, with attitude and perceived behavioural control were also significant predictors for intention. Subjective norm had less influence but a distinct role shaping intention. A total of six of 11 studies overlapped with the comparison review. Selection of different studies were due to differences in search databases, search strategy, and inclusion and exclusion criteria. Inconsistency was also found in relationships among other components in the theory, with different interpretations made between the two reviews. The current replication review provides practical and research related implications that add to the understanding of the TBP for persons living with physical disabilities and the findings of the comparative review.

    Introduction. Reperfusion therapy by primary percutaneous coronary intervention (PPCI) is generally indicated in patients suffering from acute myocardial infarction (MI) with ST-segment elevation (STEMI). Prior to hospital admission, full ST-segment resolution (fSTR) may occur. Optimal management of such patients with transient STEMI (TSTEMI) is potentially challenging. Our aim was to evaluate the hypothesis that in TSTEMI patients, patency of infarct related artery (IRA) is achieved before PPCI, and to compare the outcome of TSTEMI and STEMI patients during a prolonged follow-up. Material and Methods. Three hundred consecutive adult STEMI patients were referred to catheterization laboratory. In all patients, standard 12 lead ECGs were obtained both at the time of the first medical contact, and on catheterization laboratory admission. Results. TSTEMI occurred in 20 patients (6.7%). Despite fSTR (isoelectric ST segment), occluded IRA was found in 5 of these patients (25%). Pre-PPCI TIMI flow grade 2 was found in 6 TSTEMI patients (30%). Troponin T value at 24 h after symptom onset was lower in the TSTEMI group (1.8±2.5 mg/L vs. 3.6±3.5 mg/L, P=0.008). These patients also had a lower value of brain natriuretic peptide (156.3±119.5 ng/L vs. 438.5±429.0 ng/L, P<0.001) and higher left ventricular ejection fraction (59.9±6.3% vs. 51.6±10.2%, P<0.001). All patients were followed for a median of 5.6 years during which the overall survival did not differ between the TSTEMI and STEMI groups. Conclusion. Primary PCI is strongly recommended in TSTEMI patients because of a relatively high incidence of occluded infarct related arteries. The rate of patients with TSTEMI is relatively low.

    Muscular strength is a critical component of physical health. The aim of this cross-sectional study was to examine age- and sex-based differences in functional strength and rates of referral for education in a large international sample of Special Olympics athletes. A total of 30,358 (male = 19,661) adult (20–69 years old) athlete functional strength records were utilized from the Special Olympics Healthy Athletes database. Descriptive statistics were computed for the following tests: timed sit to stand, partial sit-up, grip strength, and seated push-up. Analyses of variance with Fisher-Hayter pairwise comparisons generally supported age-related decreases in strength among both males and females in this study. Overall, male participants were stronger than the female participants. However, significantly fewer women were referred for education based on their grip strength scores. The rates of referral for education are a concern regarding core- and leg strength for more than one-third of athletes over the age of 30. These findings suggest that a substantial proportion of Special Olympics athletes would benefit from additional strength and conditioning training. peerReviewed

    Football (soccer) is one of the most popular sports for individuals with visual impairments (VI) worldwide but nascent within the United States (US). While there is burgeoning interest toward developing blind football at grassroot and national levels, particularly with the forthcoming 2028 Paralympics, no football-based talent identification or skill assessment has been developed or vetted in the US. Therefore, the purpose of this study was to investigate the immediate measurement properties of scores from a novel football assessment in amateur-skilled adolescents with VI in the US. Adolescents (n = 57) with VI physically completed the Blind Football Skills Test for Adolescents with VI (BFST-AVI) which was comprised of five football-related tasks. Scores were analyzed by item analyses, internal consistency, convergent validity, and construct validity procedures. The results suggested that total and individual assessment scores had acceptable levels of reliability and validity. The BFST-AVI can be adopted immediately by grassroot organizations, national governing bodies, coaches, adapted and general physical educators, and associated practitioners for talent identification or general motor skill assessment purposes for amateur-skilled adolescents with VI.

    Abstract We previously created and characterized a mouse model of congenital erythrocytosis with low erythropoietin (EPO) levels from a gain-of-function mutation of the human erythropoietin receptor gene (mtHEPOR) (Divoky et al. PNAS. 2001; 98:986; Divoky et al. JMM Berl. 2016; 94:597). These mice develop fetal erythrocytosis, followed by transient amelioration of erythrocytosis in perinatal life, and reappearance at 3-6 weeks of age. Similarly, erythrocytosis is observed in heterozygous mtHEPOR patients postnatally but not at birth. We previously reported dynamic changes of the erythron with iron homeostasis during ontogenesis in these mice (Kralova et al. Blood 2017; 130: 170). We observed that while perinatal mtHEPOR mice exhibit relative iron deficiency, aged mice had iron overload. Here, we evaluated developmentally-determined factors associated with hyperactivation of EPOR signaling which could cause a transition from iron deficiency (neonates) to hyperferremia and increased iron deposition (aged mice). To assess the consequences of different levels of EPOR-JAK2-STAT5 signaling, we studied hetero- and homozygous mtHEPOR mice that differ in their severity of erythrocytosis. We found that prenatally and perinatally, mtHEPOR hetero- and homozygous mice have increased erythroferrone (Erfe) transcripts and reduced hepcidin, consistent with the known inverse correlation between Erfe and hepcidin and in accordance with increased numbers of immature erythroid progenitors in the fetal hepatic circulation. At birth, previously normal Epo expression decreased and remained low in adulthood. Iron deficiency, observed in mtHEPOR hetero- and homozygotes at postnatal day 7, was likely related to increased iron consumption by augmented erythropoiesis at this stage. Postnatally, hepcidin levels increased in mutant mice, accompanied by low Erfe induction and iron accumulation in the liver and spleen as reflected by the upregulation of hepatic Bmp6 expression in mature adult (aged ~6.5 months) and old (~16 months) mtHEPOR homozygotes. We hypothesized that this could be a consequence of diminished iron consumption due to a progressive decline of erythropoiesis in mtHEPOR mice, possibly mediated by premature aging of erythroid progenitors with cell-autonomously increased proliferative history and/or increased inflammation. Indeed, young mutant erythrocytes had decreased erythrocyte survival and expression of a senescent marker CD47, an inhibitor of erythrocytes' phagocytosis. Additionally, a progressive decline in the percentage of Ter119-positive bone marrow cells and immature erythroblasts was observed in mtHEPOR hetero- and homozygotes with aging. Clonogenic assays of old mice revealed suppression of early (BFU-E) and late (CFU-E) erythroid progenitors and myeloid bias of hematopoiesis, paralleled by the up-regulation of PU.1 expression, elevation of platelet counts, and an increase in megakaryocytes chiefly in the bone marrow of mtHEPOR homozygotes. Serum levels of inflammatory cytokines did not indicate systemic inflammation; however, induced transcripts of IL-6, Inf-γ, Tgf-β, and Tnf-α, mainly in mtHEPOR homozygotes showed local bone marrow inflammatory stress. These data indicate progressive attenuation of erythroid drive in mtHEPOR homozygotes, and less so in mtHEPOR heterozygotes, paralleled by a decline in hematocrit levels with aging. In response to attenuated erythropoietic activity, iron consumption was reduced in mtHEPOR mice, leading to iron accumulation in the liver and spleen accompanied by markedly increased hepcidin synthesis. Our data suggest that even in the absence of systemic inflammation, albeit with possible paracrine inflammatory signals, known to affect bone marrow remodeling and hematopoietic aging, life-lasting prolonged activation of EPOR-JAK2-STAT5 signaling promoted exhaustion of erythroid progenitors and resulted in an age-related decline of accelerated erythropoiesis in this mouse model of congenital erythrocytosis with human gain-of-function EPOR.
